Volunteer professionals provide outpatient services on a scheduled basis. We ask that all persons who would like to be seen to visit the Clinic Monday – Friday to pick up an application and to make an eligibility appointment. Patients must provide proof of residence in Hillsborough County, identification, and proof of Household income. Household income must fall between 100% and 200% of Federal Poverty Level. Patients who do not meet the above eligibility guidelines will be addressed on a case by case basis, or referred to an alternate source for their needs. Occasionally, a patient's needs exceed clinic capability. The patient is then referred to an appropriate specialist and/or facility and is seen at
no charge.
